Universal Application for License Renewal with Dental Hygienist Addendum; retain certificates 3 years for audit.

20 CEUs per biennium under Den 406.03(a) (adopted 10/6/2025): at least 15 CEUs in clinical dental care subjects; BLS-HCP with hands-on component (3 CEUs per biennium, counted as clinical); at least 2 CEUs infection control in a course including CDC infection prevention and control guidelines; at least 2 CEUs medical emergency training. EFDA hygienists add 10 CEUs in restorative dentistry. Up to 75% of CEUs may be online/webinar; no carryover; first biennial renewal after initial licensure is exempt. Retain documentation 3 years for audit.
