Reporting
Affidavit affirming CE completion filed with the renewal application; retain documentation of attendance for Board audits.

45 credit hours of recognized continuing dental education per triennial period under A.A.C. R4-11-1204: at least 25 hours in clinical/core areas; no more than 11 hours in practice organization/management; at least 3 hours each in (1) chemical dependency/tobacco cessation/ethics/risk management/Arizona dental jurisprudence, (2) infectious diseases/infection control, and (3) CPR healthcare provider level/ACLS/PALS. Self-study and similar independent activities (correspondence, internet, teaching, publication, peer review, board service, charitable clinic service) are capped at 15 of the 45 hours under R4-11-1209(B)(3). Renewal is by affidavit affirming CE compliance (R4-11-1202); credits from one renewal period may not be carried forward, and the Board audits compliance randomly. A current CPR/ACLS/PALS certificate is required before receiving a renewal license. Licensees in their first year after initial licensure or on disability status are exempt from the affidavit. Renewal deadline is the last day of the birth month every third year under A.R.S. 32-1287.
