Self-report on renewal application; retain certificates for audit

Verified against 16.19.4.10 NMAC (RLD July 2025 PDF), 16.19.26 NMAC as amended effective February 10, 2026 (New Mexico Register Vol. XXXVII Issue 3), and 16.19.4.17 NMAC (pharmacist clinician, via Cornell LII): 30 hours every two years with at least 10 live hours (law hours excluded), 2 patient-safety hours, 2 New Mexico Board pharmacy-law hours, and 2 safe/appropriate opioid-use hours; a two-hour combined patient-safety/opioid course may satisfy both. The renewal period runs the 24 months from the last day of the pharmacist's birth month (NM Register XXXV/21 amendment); initial New Mexico licensees are exempt for the initial period. Prescriptive-authority CE under 16.19.26 now counts toward the base 30 (the 'in addition to' language was removed): vaccines - 1 live ACPE hour (16.19.26.9.B(3)); tobacco cessation - 1 ACPE hour (16.19.26.10.B(3)); opioid antagonist - 1 ACPE hour (16.19.26.12.B(3)); hormonal contraception - 1 live ACPE hour (16.19.26.13.B(3)); point-of-care testing - 2 live ACPE hours per category exercised (16.19.26.14.B(3)); TB-testing CE is 'as appropriate to maintain competence' with no fixed hours. Pharmacist clinicians must complete 20 additional contact hours beyond the base 30, at least 10 live (16.19.4.17.C(2)(a)), plus 2 hours per renewal period in responsible opioid prescribing when holding a schedule II or III controlled-substance registration (16.19.4.17.C(2)(b)).
