Self-report on renewal application; retain certificates for audit.

30 hours of qualified continuing professional education per two-year licensure period under Utah Admin. Code R156-69-304a: unlimited classroom courses in blocks of at least 50 minutes; caps of 10 hours for teaching, 15 hours for online/home study (with exam and completion certificate), and 3 hours for practice and office management; up to 15% may be earned through direct patient-care volunteer services at 4 volunteer hours per CE hour. CE must be relevant to the licensee's practice, presented by qualified individuals, and verified for attendance. A current CPR-BLS, ACLS, or PALS certification (as required by the license/permit classification) is required to renew, but its recertification hours do not count toward the 30. CE documentation must be retained for two years after the end of the renewal cycle. License renewal date May 31 of even-numbered years per R156-1-308a.
