Affidavit of completed hours with renewal application; retain certificates for audit.

36 hours per three-year licensing period under 225 ILCS 25/16.1 and 68 Ill. Admin. Code 1220.440: at least 50% (18 hours) must be face-to-face, in-person instruction. Current BLS certification is required and counts for no more than 4 hours toward the CE total. Renewal requires an affidavit of completed hours. Volunteer clinical hours (up to 4 per period) may count.
