Failure to renew by the surname-based deadline: the license expires and the holder is not licensed to practice. Expired 6 months or less: may renew by paying the renewal fee plus a $100 late charge. Expired more than 6 months: must reapply and pay the application fee; the Board may require an appearance, a written or oral examination, and/or proof of 30 PDH completion. CE deficiency: renewal is denied if proof is not submitted or the deficiency is not cured.Verify with board
