The DOPL APRN renewal/reinstatement form states reinstatement costs the renewal fee plus $20 within 30 days of expiration, or plus $50 when more than 30 days and within two years of expiration; failure to reinstate within two years requires a new application. DOPL notes fees are subject to change each July 1, so the 2022-dated schedule should be rechecked at renewal time.
