Failure to renew on or before October 1 each year constitutes forfeiture of the optometrist's license. The board may, in its discretion, renew the license upon application and payment of the required fee plus a board-set late fee not exceeding $100 per month the renewal is late.Verify with board

CE courses must be COPE-certified or board-approved; attendance must be submitted by each optometrist when renewing the license; the board may waive any or all CE in case of certified illness or undue hardship.
