Licenses expire annually on the last day of the pharmacist's birth month. The renewal fee doubles if not received within 15 days after expiration (OAC 535:1-11-5(6)). A pharmacist who fails to remit the fee by the 15th day after expiration pays the Board-set late fee; after 30 days following notice the Board may deprive the person of the license, and regaining licensure requires written application for reinstatement to the Board.
