Per HAR 16-95-72, failure to timely renew by Dec 31 of odd-numbered years causes automatic forfeiture of the license. A forfeited pharmacist license may be restored within 3 years upon written application and payment of the biennial renewal fee, a $50 penalty fee, and Compliance Resolution Fund assessments.
