A license not renewed by June 30 of the renewal year is forfeited; forfeited licenses may be restored within 2 years ($226 restoration fee, including a $30 penalty over the $196 on-time renewal). After 2 years the license terminates and the nurse must apply as a new applicant. Practicing on a forfeited license is illegal/unlicensed activity punishable to the full extent of the law.Verify with board
